Required Documents
Necessary documents in order to apply for a:

TOURIST VISA

1. Application form duly completed and signed by the applicant.
2. Two recent color photos (size 35mm x 45mm with white or very light background and in accordance with the I.C.A.O. standards).
3. Valid passport (valid for at least 3 months beyond the last day of the intended stay in the Schengen area). This passport should not bear any correction regarding the personal identification data. If your passport contains a correction regarding this personal data, you are required to change your passport with a new passport that contains correct data. NO additional correction on page 4 or 5 is allowed.
4. Copy of passport’s bio-data page. For non-Indonesians, please provide a copy of residence permit in Indonesia (KITAS/KIMS/KITAP).
5. Travel insurance for the whole period of stay (please see Note A).
6. Employment letter stating the applicant’s position of work, duration of employment and the confirmation to the effect that the applicant has been granted leave for the period corresponding to the stay in Schengen area and is expected to report back on duty. Please also attach one copy of the company registration (i.e. SIUP or TDP).
7. Bank Reference Letter and an authenticated copy of recent personal bank statement over the last three months.
8. Original or copy print-out of confirmed flight-booking or air ticket from travel agent/airline. It should prove that the stay in the Schengen area does not exceed 90 days.
9. Copy of confirmed hotel voucher/hotel reservation for the whole period of stay. In case of stay in a private house please see note B.
10. In case of children of minor age (under 18 years old) travelling alone, or with one parent only, a letter of consent is required from both parents/legal guardians or from the parent who does not travel. This consent letter, which will be provided at the Italy Visa Application Center, must be signed by the parent(s) directly in front of the official who receives the visa application (with attached copy of ID Card of parents and birth certificate).
11. Copy of the Family Card and Marriage Certificate/Birth Certificate (when traveling with family).

For spouse of European Union citizen, documents required are as follow:

1. Application form duly completed and signed by the applicant.
2. Two recent color photos (size 35mm x 45mm with white or very light background and in accordance with the I.C.A.O. standards).
3. Valid passport (valid for at least 3 months beyond the last day of the intended stay in the Schengen area). This passport should not bear any correction regarding the personal identification data. If your passport contains a correction regarding this personal data, you are required to change your passport with a new passport that contains correct data. NO additional correction on page 4 or 5 is allowed.
4. Copy of passport’s bio-data page. For non-Indonesians, please provide a copy of residence permit in Indonesia (KITAS/KIMS/KITAP).
5. Copy of the Marriage certificate issued by the Town hall of a Country in the European Union or a copy of acknowledgement/registration of the marriage at the respective Embassy in Jakarta.
Should the marriage not be registered in the EU country or at the respective Embassy, please be advised that it is then required to submit the documents as a regular tourist visa.
6. Original or copy print-out of confirmed flight-booking or air ticket from travel agent/airline. It should prove that the stay in the Schengen area does not exceed 90 days.

NOTE

A. Travel Insurance:
  All applicants must submit copy of proper travel medical insurance at submission time.
The insurance policy must contain the following information:
 
Name of person covered and period of validity
Coverage of medical expenses up to equivalent of Euro 30000
Coverage of the entire Schengen area - not just Italy
Coverage of the Entire Period of stay in Schengen (as indicated on flight ticket)
  Note also that the insurance coverage may be checked at the border of entry in Schengen. Insufficient proof of insurance is one of the grounds for refusal of entry.
B. If invited/sponsored by Italian citizens or foreigners holding a valid permit of stay in Italy (in the absence of the requirements listed above), the following documentation is required:
 
Bank or Insurance Fidejussione (in case of insufficient own financial means). Please bring the original and one photocopy of the original. The Embassy will keep the photocopy and return you the original directly when you submit the application after you put your signature on the photocopy for receipt.

On April 5, 2010, regulation no. 810/2009 of the European Parliament and of the Council establishing a Community Code on Visas (Visa Code) enters into force.

These new regulations imply new demands on the application, meaning that all supporting documents must be handed in along with the Schengen application form.
If supporting documents are not presented at the same time as the application, your application upon examination may be refused and the visa fee lost.

Processing Time
The visa fee is collected in cash at the VFS application centre at the time of handing in the application and is not refundable.

Submission at the Visa Application Centre - it is mandatory to schedule an appointment to submit your Short Stay application.

Step 1

Step 2
Book an appointment to visit Visa Application Centre (Mandatory): To avoid waiting time due to high volume of applicants applying at the same time, we strongly recommend you book an appointment before visiting the Visa Application Centre. Booking and appointment is FREE OF CHARGE and is subject to availability. To book an appointment, please click here.

Step 3
Submit your application: Visit Visa Application Centre as per appointment scheduled and submit your completed Visa Application Forms along with required supporting documents.

Step 4
Pay applicable Fee: Please check ‘Visa Fee’ table provided under the visa category you are applying for. Visa Application Centre accepts the fee in CASH ONLY. Submission Officer at the Visa Application Centre will advise you on total payable amount in local currency only (IDR). A receipt will be provided for all payments made at the Centre.
