Short Term Visa
If you wish to visit the Czech Republic for a short period of time, you must obtain a visa prior to entry if you come from a country with a visa requirement for entering the Schengen area. Ukrainian nationals are required to have a visa to the Czech Republic.
A short-term visa is only intended to allow a foreign national to visit the Czech Republic for a limited period of time (90 days in any 180 day period). If you wish to reside in the Czech Republic for an extended period of time (more than 90 days), you need to apply for a long term visa / residence permit directly at the Consulate of the Czech Republic.
All customer would need to appear in person in order to provide biometric data (fingerprints and digital photography). This concerns the applications for the short-term Schengen visas (max. 90 days in 180 days).
For subsequent applications within 5 years the fingerprints can be copied from the previous application file in the VIS, eg. the fingerprints will be provided for the period of 5 years for any subsequent Schengen visa application (for example for the long-term multiple entry visa).
Exemptions from the obligation of fingerprinting are provided for:
- children under the age of 12;
- persons for whom the collection of fingerprints is physically impossible;
- Heads of State and members of the national Governments (with their official delegations and spouses) if they travel for official purposes.
Below you can find a list of existing visa categories:
- Tourism: This type of visa applies to those customer who are traveling to the Czech Republic for the purpose of tourism.
- Visit to family or friends: This visa is eligible for customer who wish to visit relatives or friends who legally reside in the Czech Republic.
- Business: This visa category is suitable for customer who are traveling for the purpose of business (commercial purpose).
- Official visit: This visa is eligible for customer who wish to visit the Czech Republic for official purposes. Those customer belong to this category who are the members of official delegations and who plan to participate in meetings, consultations, negotiations or exchange programs.
- Sports: This type of visa can be used by the customer who take part in sports events in the Czech Republic, as well as persons accompanying them for professional reasons.
- Scientific, cultural, and artistic events: This visa category is designed for those customer who wish to visit the cultural, scientific or artistic events, which will be held in the Czech Republic.
- Medical treatment: This type of visa is eligible for customer who wish to visit the Czech Republic for medical treatment.
- Study: This visa category is designed for those customer who wish to visit the Czech Republic for study or training purposes.
- Visa for drivers: This category of visa caters for drivers conducting international cargo and passenger transportation services in vehicles registered in Ukraine.
- Visa for journalists: This visa category is eligible for customer who are qualified journalists and travel to the Czech Republic with the aim of journalism.
- Czech compatriots: This visa category belongs to customer who are the citizens of Ukraine with Czech roots.
- Employment up to 90 days: This visa category is designed for those customer who wish to work in Czech Republic for a limited period of time of not more than 90 days in any 180 day period.
According to the decision taken by the Ministry of Foreign Affairs of the Czech Republic, effective since 18th of April 2011 third countries where the Czech Republic has a number of representative bodies, customer may apply for a visa only at the representative body within the consular district, in which they have been admitted for permanent or temporary residence.

According to the law of Ukraine each applicant must sign the Personal Data Information Protection Agreement. For minors this agreement has to be signed by one of the parents or a legal guardian. Long Term Visa
Effective 01 May 2018 Visa Application Centre VFS Global in Lviv will start accepting documents from customer which are included to the Czech government program «Rezhym Ukraina» for issuing employee cards. customer with scheduled appointment date only will be able to submit their application.
customer without appointment will not be able to submit their documents for issuing employee card in Visa Application Centre.
Applications for issuing employee card will be accepted in Visa Application Centre in Lviv only at the address: 18 Dzherelna Str, Lviv. There is no possibility to submit documents for this category of visa in other Visa Application Centres.
Needful documents for submission, processing time and other rules remain the same and are available also on the web-site of Consulate General of Czech Republic in Lviv at www.mzv.cz/lvov.
Visa Application Centre accepts documents for a work card only according to the date of registration, and only between 08:30 and 12:00. If the client comes to submit documents to the visa center after 12:00, he will be refused a visa application.
In case applicant is not able to visit the VAC for documents submission without any sound reasons, he/she should contact directly his/her employer in Czech Republic and not VFS Global or Consulate General of Czech Republic in Lviv in order to be included again into governmental program by employer if he is still interested in applicant's employment.
Appointment is being scheduled by the Consulate General of Czech Republic in Lviv only and strictly according to the order received from the Ministry of Foreign Affairs of Czech Republic. Applicant should visit Visa Application Centre strictly on the scheduled by the Consulate date. There is no possibility to change appointment date.
Employees of Consulate General of Czech Republic in Lviv will be present at the VAC during documents submission.
Please note that Consulate General of Czech Republic in Lviv strongly recommends accepting documents from customer even in case of missing one or more documents, but applicant will be issued a written notice with all missing documents and timelines (as a rule within 60 days period) during which he/she will have a possibility to submit them.
Attention: time of documents processing starts after the last missing document is submitted.
Resubmission of missing documents is possible at the Consulate General of Czech Republic only at the address: 130 A Antonovycha Str, Lviv during the working hours of the consulate. All missing documents must be provided in the original or notarized copy, personally or being sent by post. They cannot be sent via e-mail.

Longterm Stay - Employee Card

Customer without appointment will not be able to submit their documents for issuing employee card in Visa Application Centre.
Applications for issuing employee card will be accepted in Visa Application Centre in Lviv only at the address: 18 Dzherelna Str, Lviv. There is no possibility to submit documents for this category of visa in other Visa Application Centres.

Visa Application Centre and Consulate General of Czech Republic in Lviv strongly recommend customer to submit their documents according to the scheduled appointment date within 08:30 – 12:00 timelines. In case an authorized representative provides missing documents, he/she should additionally present letter of authority on behalf of a customer (should be signed by customer and contain contact phone number of representative), and also ID card of representative in original and copy. Documents cannot be sent via e-mail or post to the Consulate General of Czech Republic in Lviv. Customer should bring notice from CG along with all missing documents.
In case customer is not able to submit missing documents due to objective causes, he/she has to inform about this before the expiration of this period personally or send by post to Consulate General of Czech Republic in Lviv (address – Lviv, 130 A Antonovycha Str) an explanation letter in the Czech language with the request to prolong this period. This request cannot be sent via e-mail

Visa Fees

Documents Required

Application form for employee card
Must be filled in and signed personally by the customer
- Passport
- Passport must be submitted in the original (including other valid passports).
- Copy of all pages with notes and previous visas.
- Not older than 10 years (extended passports are not accepted).

Validity of passport should be at least 3 months after the expected visa expiration. Visa with the purpose of obtaining residence permit is issued to customer for 60 days within period of 180 days.
Example of passport validity calculation is below: day of documents submission with complete set+period from 60 to 90 days for decision making process regarding issuance of residence permit by Ministry of Internal Affairs of Czech Republic + period from 1 to 14 days for collection of visa for employee card issuance at CG of Czech Republic in Lviv + 180 days of the issued visa.
- Should not be damaged.
- Passport must have at least two (2) blank pages.

- Confirmation of accommodation on the territory of Czech Republic

Contract of employment, an agreement on work activity or an agreement for a future contract
(in this case customer should submit both original and notarized copy of the document; original is returned back at the moment of documents submission). Parties agree to enter into a contract of employment or an agreement on work activity by an agreed deadline. The aforementioned documents must contain a stipulation which provides that regardless of the scope of work, the agreed monthly salary will not be lower than the basic monthly minimum wage and that weekly working hours will be at least 15 hours.
These documents can be substituted by the letter in case of foreign customer which are transferred to work on the territory of Czech Republic by the foreign employer and do not conclude agreement with domestic employer. Foreigner must provide these documents in Czech language and translation should be notarized, clear and understandable to foreigner or should be translated into language which is clear to a foreigner with notarized translation into Czech language.

Criminal Records Certificate in Ukraine (Full)
In case a foreigner lived constantly in any country for 6 months or more in the previous three years, Consulate General has a right to ask for submission of a Criminal History Record issued by that country. Here the crucial fact is that residence permit in another country is issued for more than 6 months, and not the period of customer's stay in this country).
In case customer changed his/her surname or name in the past, he/she should provide certificate for all previous surnames/names.

Document proving the purpose of stay
In this case customer must submit both original and notarized copy; original is returned back to the customer while documents submission:
- Documents proving the professional qualifications for performance of the desired job.
- Work permit if needed.
- Document confirming about employment of a foreigner according to Article §98 of Law on employment.
- Document proving that you meet the requirements for performing an occupation referred to as "regulated occupation", if your application concerns this kind of occupation.
Work permit abroad according to the Law on Employment no. 435/2004 Sb., demands the following:
- In case a foreigner whose employer is a foreign juridical entity and who is employer on the ground of agreement with Czech juridical entity or is a physical person which is supposed to perform working activity on the territory of Czech Republic.
- Or in case if foreigner is going to be employed as seasonal worker but for the period of not more than 6 months in the calendar year in, if the period of not less than 6 months will be between two different working activities; who will be employed for the whole period in order to improve his/her skills and qualification in the chosen activity (probation) but for the period not longer than 6 months. This period can be prolonged, but not more than the period needed for obtaining professional qualification as per legislation in Czech Republic.
Important information:
Entry visa for getting residence permit is issued for the period of 6 months with up to 60 days of allowed stay in Czech Republic. Upon arrival to Czech Republic, foreigner should personally visit an office of the Ministry of the Interior in order to submit application for residence permit issuance, but not later than within 3 working days. Such residence permit will be issued within 60 days.
Valid residence permit allows its holder to stay up to 90 days on the territory of other Schengen countries apart of Czech Republic within 180 days period.
All submitted documents for cannot be older than 180 days, except for the travel passport, matrix documents and photos (if they show the current appearance of the applicant).
All documents in a foreign language must be submitted with an officially certified translation into the Czech language. In case of submission of Ukrainian documents apostil is not required. It will be enough to submit such documents being notarized.
Customer should pay visa and service fees.
Application for obtaining employee card is being processed by Ministry of Internal Affairs and may take time generally to 60 days from the moment of submission date, in more complex cases within 90 days.
If case processing is being suspended, the period is not counted (e.g. due to necessity of submitting missing documents).
Generally, it is issued for the duration of the employer-employee relationship but not for more than 2 years, with an option to repeatedly extend its validity. In case an customer submits work permit, employee card is issued for the period which is indicated in this work permit.
Showing the current appearance of the customer.
Relevant document should prove that accommodation will be provided to customer for the period of the needful stay from - to or indefinitely. This document can be:
Original or notarized copy of accommodation agreement, rent agreement, subtenancy contract or contracts with the similar content. In such cases there is no need to certify signatures of both sides (applicant can ask to return this document on the stage of application submission) or
Prove from physical party which is authorized to act on behalf of juridical party, or which is the owner or authorized habitant of the house/apartment
Or
Certificate issued to foreigner on the owner's request - in this case signatures should be certified.
Details of article d) of the Act No. 326/1999 Coll. on the Residence of Foreign Nationals in the Czech Republic:
Landlord is obliged to ensure that foreigner's accommodation that is not manifestly unreasonable level accommodation provided in buildings other owners of similar destination in the village or district or region. Comparison of levels of accommodation is done in particular by examining the adequacy of number of persons with regard to secured sanitary conditions, and the floor area of the room for accommodation, which must be at least 1) 8 m 2, it is accommodated by one person, 2) 12.6 m 2 when two people stay, to any other person accommodated in the floor area of 5 m 2 is added.

Photo Specifications
A Photograph submitted with the application for a visa should comply with the following specifications:
- Must be an up-to-date photograph (preferably maximum 6 months old)
- Must be a colour photograph printed on normal photographic paper
- Should be passport sized 3,5 x 4,5 cm
- Close up to your head and top of your shoulders so that your face takes up 70-80% of the photograph
- Taken against a light background
- Without sunglasses
- Without hat or other head covering unless you wear this for religious reasons
- Spectacles should not have toned or reflecting glasses
- The frame of the spectacles may not cover the eyes
- The mouth must be closed

Processing Time
Visa processing time takes 60 days (in complex cases up to 90 days). In case customer does not provide full set of needful documents he is issued a notice for their resubmission within the 60 days period.
Please note that period of 60 or 90 days of application processing starts only after last required document which is missing is being submitted.
The decision about visa application is taken by the Ministry of Internal Affairs of Czech Republic.
Only Ministry of Internal Affairs of Czech Republic possesses the information regarding application processing. Hence we kindly ask customer not to refer neither Consulate General of Czech Republic nor to the Visa Application Centre with the questions regarding the stage of case processing or whether decision is already made.
As soon as the Consulate General receives decision from Ministry of Internal Affairs of Czech Republic, customer is informed about it straight away via sms on his mobile phone number or via post.

Visa issued with the purpose of obtaining residence permit (employee card) can be collected at the Consulate General of Czech Republic in Lviv only (130 A Antonovycha Str, Lviv) during official working hours of the Consulate which you can find at www.mzv.cz/lvov.
Customer will be informed regarding date of visa collection via the letter from Consulate sent by post or via sms to his mobile phone number. This letter or proved sms message must be provided to the Consulate employee on the defined date. If the customer is not able to visit Consulate for visa collection on the defined date, he can do this in the nearest time providing the Consulate employee the letter or sms message which was sent to him regarding visa collection.

Tourism

Overview
This type of visa is applicable for those who tend to visit the Czech Republic for tourism.
It allows the citizens of Ukraine travel to the Czech Republic and stay for a limited period of time of not more than 90 days in any 180 day period.

0 As per the Visa Facilitation Agreements between the EU and Ukraine the following citizens of Ukraine are exempted from the visa fees:
- close relatives (spouses, children (including adopted), parents (including custodians) of citizens of Ukraine legally residing in the territory of the Member States or citizens of the European Union residing in the territory of the Member State of which they are nationals;
- members of official delegations, who following an official invitation shall participate in meetings, consultations, negotiations, or exchange programs, as well as events held in the territory of one of the Member States by intergovernmental organizations;
- members on national and regional Governments and Parliaments, Constitutional Courts and Supreme Courts;
- pupils, students, postgraduate students and teachers accompanying them when they come to attend school, study or educational training;
- disabled persons and the person accompanying them, if necessary;
- persons travelling on humanitarian grounds (including urgent medical treatment) and the person accompanying, or to attend a funeral of a close relative, or to visit a seriously ill close relative;
- participants in scientific, cultural and artistic activities including university and other exchange programs;
- participants in official exchange programs organized by twin cities and other municipal entities;
- journalists and the technical crew accompanying them in a professional capacity;
- pensioners;
- drivers conducting international cargo and passenger transportation services to the territories of the Member States in vehicles registered in Ukraine;
- members of train, refrigerator and locomotive crews in international trains, travelling to the territories of the Member States;
- representatives of the religious communities;
- members of the professions participating in international exhibitions, conferences, symposia, seminars or other similar events held in the territory of the Member States;
- representatives of civil society organizations undertaking trips for the purposes of educational training, seminars, conferences, including in the framework of exchange programs;
- participants in official European Union cross-border cooperation programs, such as under the European Neighborhood and Partnership Instrument (ENPI);.
- participants in international sports events and persons accompanying them;
- children under the age of 18 and dependent children under the age of 21;
- participants aged 25 years or less in seminars, conferences, sports, cultural or educational events, organized by non-profit organizations.
Other nationalities:
- citizens of all other countries under 6 years of age.
- The visa fee is payable in Euros at the bank after documents submission. The bank charges the additional fee of UAH 45,00 for the bank services for each applicant.

Documents Required
Please note that as per the Embassy's requirement effective 1 April 2015 the Czech Republic Visa Application Centre will be not in a position to accept incomplete set of documents. Please prepare the full set of documents in advance as per the chosen visa category.
For Ukrainian citizens and other nationals legally residing in the territory of Ukraine, who are applying for the visa category "Tourism" the list of required documents is as follows:

Visa application form.
- Completed and signed personally by the applicant (for minors the application has to be signed by one of the parents or a legal guardian).
- Filled in Latin letters.

Passport or other travel document.
- Passport must be presented in the original (including other valid passports).
- A copy of all pages with notes and previous Schengen visas.
- Not older than 10 years (extended passports are not accepted).
- Validity of passport should be at least 3 months after the visa expiration.
- Passport must have minimum two (2) blank pages.
- Original Internal passport and copy of all the pages with notes or original ID card and copy of both sides and original and copy of the Registration of Individual.

Original medical insurance policy.
- Coverage of 30,000 Euro.
- Franchise for medical expenses – 0 Euro.
- The grounds of action - all European Union countries.
- In case of applying for multiple-entry visa, the insurance policy must cover at least the first trip. In this case the visa application form should contain a third signature that certifies the necessity of having insurance policy for all future trips.
- In case of applying for a single entry visa, insurance should cover additional 15 days.

- For employees - original certificate of employment, which contains the position, salary for the last 6 months, hire date and approved holiday period. The certificate shall indicate the name and position of the person who signed the certificate, registration and contact details of the company, also certificate should be certified by wet seal. Additionally, you may need to provide work book.
- For entrepreneurs - license, registration certificate and latest tax declaration;
- For pensioners - pension certificate or document certifying the size of pension;
- For students and school children - confirmation from educational establishment with confirmation of leave of absence. Additionally, you may need to provide a student's record-book.
- For unemployed - a written signed explanation on the applicant's labor situation. Additionally you may provide a copy of work book with the previous official places of employment.

Proof of sufficient funds to cover expenses during the trip (only one proof is required).
- Bank account which confirms the adequacy of funds and transactions over the past 3 months. The bank statement must be submitted in the original and be stamped by the bank.
- Signed traveller's checks on applicant's name - the original and copy.
- Sponsorship - notary sponsorship letter, covering the entire period of the requested visa and bank transactions of a sponsor. For children and spouses the declaration is also required.

Additional documents for children under 18 years of age traveling alone or accompanied by only one parent.
- Written consent of one / both parents (if they are not traveling with a child) - the document must be notarized, submitted in original and copy, and should cover the entire period of the requested visa. If there is only one guardian it is required to provide a birth certificate, a judgment on custody, a death certificate or other document which confirms the reality.
- A copy of a foreign or internal parent's passport.

Citizens of other countries are required to submit a registration or a residence permit on the territory of Ukraine. This document must be valid for at least 3 months after the expiry of the visa.
According to the Law of Ukraine "On Personal Data" agreement is signed personally by the applicant (for minors signature poses one of the parents or legal guardians). Please note that customer who apply through the representative must also prepare this document in advance.
In case of absence of this document the Visa Application Centre has the right to refuse to accept documents.

A Photograph submitted with the application for a visa should comply with the following specifications:
- Must be an up-to-date photograph (preferably maximum 6 months old)
- Must be a colour photograph printed on normal photographic paper
- Should be passport sized 3,5 x 4,5 cm
- Close up to your head and top of your shoulders so that your face takes up 70-80% of the photograph
- Taken against a light background
- Without sunglasses
- Without hat or other head covering unless you wear this for religious reasons

Processing Time
Application for a Schengen (short-term) visa can be filed within 3 months preceding the planned trip. Applications are in most cases reviewed within 7-10 calendar days. In individual cases, the review period can be extended up to 45 days and more at the discretion of the Consulate of the Czech Republic for any reason. It is recommended not to file a visa application later than 15 days before the trip, as it cannot be otherwise guaranteed it will be reviewed in time. Please note that the processing time in General Consulate of Czech Republic in Lviv ( Visa Application Centre in Lviv, Uzhgorod and Ivano Frankivsk) will take 10 working days and not include 2 days for transportation the application from Visa Application Centre to General Consulate and 2 days for transportation the decision from the General Consulate to the Visa Application Centre.
Visa Application Centre has no right to influence anyhow the processing time of visa applications as well to accelerate the application.

The VAF has to be filled out in Ukrainian language with English transliteration or in English or Czech languages. The form needs to be signed personally by the applicant in the field 37 and on the last page. In case of the applications for multiple-entry visas there has to be a signature also under the declaration about the necessity to have travel health insurance not only for the first trip but for all the following trips.

Overview
This visa category is suitable for customer who are traveling for the purpose of business (commercial purpose) as well as representatives of business organizations.
It allows the citizens of Ukraine to travel to the Czech Republic and stay for a limited period of time of not more than 90 days in any 180 day period.
All customer would need to appear in person in order to provide biometric data(fingerprints and digital photography), which is a simple and discreet procedure. This concerns the applications for the short-term Schengen visas (max. 90 days in 180 days).
For subsequent applications within 5 years the fingerprints can be copied from the previous application file in the VIS, eg. the fingerprints will be provided for the period of 5 years for any subsequent Schengen visa application (for example for the long-term multiple entry visa) .
Exemptions from the obligation of fingerprinting are provided for:
- children under the age of 12;
- persons for whom the collection of fingerprints is physically impossible;
- Heads of State and members of the national Governments (with their official delegations and spouses) if they travel for official purposes.
According to the decision taken by the Ministry of Foreign Affairs of the Czech Republic, effective since 18th of April 2011 third countries where the Czech Republic has a number of representative bodies, customer may apply for a visa only at the representative body within the consular district, in which they have been admitted for permanent or temporary residence.

The Invitation letter should contain the following information about the Invited person: Name and Surname, Date of Birth, Travel Dates, Passport number, Relationship to the Inviting person and proof of Residence of the Inviter on the territory of Czech Republic.
- For those who are visiting a close relative, Citizen of Ukraine legally residing on the territory of Czech Republic, an Invitation letter should be notary authorized or issued by the Police in Czech Republic. The Invitation letter should contain the following information of the Invited person: Name and Surname, Date of Birth, Travel Dates, Passport number and proof of family ties to the Invited person (i.e. Copy of the Residence permit of the Inviter, Marriage Certificate, Birth Certificate, etc).

- For those who are visiting close relatives EU, EES or Switzerland, proof of relative ties is required (Marriage Certificate, Birth Certificate, Death Certificate of the invited person).

In case of absence of this document the Visa Application Centre has the right to refuse to accept documents.
According to the Law of Ukraine "On Personal Data" agreement is signed personally by the applicant (for minors signature poses one of the parents or legal guardians). Please note that customer who apply through the representative must also prepare this document in advance.

Photo Specifications
A Photograph submitted with the application for a visa should comply with the following specifications:
- Must be an up-to-date photograph (preferably maximum 6 months old)
- Must be a colour photograph printed on normal photographic paper
- Should be passport sized 3,5 x 4,5 cm
- Close up to your head and top of your shoulders so that your face takes up 70-80% of the photograph
- Taken against a light background
- Without sunglasses
- Without hat or other head covering unless you wear this for religious reasons
- Spectacles should not have toned or reflecting glasses
- The frame of the spectacles may not cover the eyes
- The mouth must be closed

Processing Time
Application for a Schengen (short-term) visa can be filed within 3 months preceding the planned trip. Applications are in most cases reviewed within 7-10 calendar days. In individual cases, the review period can be extended up to 45 days and more at the discretion of the Consulate of the Czech Republic for any reason. It is recommended not to file a visa application later than 15 days before the trip, as it cannot be otherwise guaranteed it will be reviewed in time. Please note that the processing time in General Consulate of Czech Republic in Lviv ( Visa Application Centre in Lviv, Uzhgorod and Ivano Frankivsk) will take 10 working days and not include 2 days for transportation the application from Visa Application Centre to General Consulate and 2 days for transportation the decision from the General Consulate to the Visa Application Centre.
Visa Application Centre has no right to influence anyhow the processing time of visa applications as well to accelerate the application.

Visa application form has to be completed for all the customer including minors electronically on your computer or in writing. The VAF has to be filled out in Ukrainian language with English transliteration or in English or Czech languages. The form needs to be signed personally by the applicant in the field 37 and on the last page. In case of the applications for multiple-entry visas there has to be a signature also under the declaration about the necessity to have travel health insurance not only for the first trip but for all the following trips.
For minors, the application must be signed by one of the parents or a legal guardian.

Overview
This type of visa can be used by the customer who plan to study in the Czech Republic.
It allows the citizens of Ukraine to travel to the Czech Republic and stay for a limited period of time of not more than 90 days in any 180 day period.
All customer would need to appear in person in order to provide biometric data(fingerprints and digital photography), which is a simple and discreet procedure. This concerns the applications for the short-term Schengen visas (max. 90 days in 180 days).
For subsequent applications within 5 years the fingerprints can be copied from the previous application file in the VIS, eg. the fingerprints will be provided for the period of 5 years for any subsequent Schengen visa application (for example for the long-term multiple entry visa) .
Exemptions from the obligation of fingerprinting are provided for:
- children under the age of 12;
- persons for whom the collection of fingerprints is physically impossible;
- Heads of State and members of the national Governments (with their official delegations and spouses) if they travel for official purposes.
According to the decision taken by the Ministry of Foreign Affairs of the Czech Republic, effective since 18th of April 2011 third countries where the Czech Republic has a number of representative bodies, customer may apply for a visa only at the representative body within the consular district, in which they have been admitted for permanent or temporary residence.

- The visa fee is payable in Euros at the bank after documents submission. The bank charges the additional fee of UAH 45,00 for the bank services for each applicant.

A Photograph submitted with the application for a visa should comply with the following specifications:
- Must be an up-to-date photograph (preferably maximum 6 months old)
- Must be a colour photograph printed on normal photographic paper
- Should be passport sized 3,5 x 4,5 cm
- Close up to your head and top of your shoulders so that your face takes up 70-80% of the photograph
- Taken against a light background
- Without sunglasses
- Without hat or other head covering unless you wear this for religious reasons

Processing Time
Application for a Schengen (short-term) visa can be filed within 3 months preceding the planned trip. Applications are in most cases reviewed within 7-10 calendar days. In individual cases, the review period can be extended up to 45 days and more at the discretion of the Consulate of the Czech Republic for any reason. It is recommended not to file a visa application later than 15 days before the trip, as it cannot be otherwise guaranteed it will be reviewed in time. Please note that the processing time in General Consulate of Czech Republic in Lviv ( Visa Application Centre in Lviv, Uzhgorod and Ivano Frankivsk) will take 10 working days and not include 2 days for transportation the application from Visa Application Centre to General Consulate and 2 days for transportation the decision from the General Consulate to the Visa Application Centre.

The VAF has to be filled out in Ukrainian language with English transliteration or in English or Czech languages. The form needs to be signed personally by the applicant in the field 37 and on the last page. In case of the applications for multiple-entry visas there has to be a signature also under the declaration about the necessity to have travel health insurance not only for the first trip but for all the following trips.

Overview
This visa category is designed for those customer who wish to visit the cultural, scientific or artistic event, which will be held in the Czech Republic.
It allows the citizens of Ukraine to travel to the Czech Republic and stay for a limited period of time of not more than 90 days in any 180 day period.
All customer would need to appear in person in order to provide biometric data(fingerprints and digital photography), which is a simple and discreet procedure. This concerns the applications for the short-term Schengen visas (max. 90 days in 180 days).
For subsequent applications within 5 years the fingerprints can be copied from the previous application file in the VIS, eg. the fingerprints will be provided for the period of 5 years for any subsequent Schengen visa application (for example for the long-term multiple entry visa) .
Exemptions from the obligation of fingerprinting are provided for:
- children under the age of 12;
- persons for whom the collection of fingerprints is physically impossible;
- Heads of State and members of the national Governments (with their official delegations and spouses) if they travel for official purposes.
According to the decision taken by the Ministry of Foreign Affairs of the Czech Republic, effective since 18th of April 2011 third countries where the Czech Republic has a number of representative bodies, customer may apply for a visa only at the representative body within the consular district, in which they have been admitted for permanent or temporary residence.

A Photograph submitted with the application for a visa should comply with the following specifications:

Processing Time

The VAF has to be filled out in Ukrainian language with English transliteration or in English or Czech languages. The form needs to be signed personally by the applicant in the field 37 and on the last page. In case of the applications for multiple-entry visas there has to be a signature also under the declaration about the necessity to have travel health insurance not only for the first trip but for all the following trips.
For minors, the application must be signed by one of the parents or a legal guardian.

Medical Treatment

Overview
This type of visa is eligible for customer who wish to visit the Czech Republic for medical treatment.
It allows the citizens of Ukraine to travel to the Czech Republic and stay for a limited period of time of not more than 90 days in any 180 day period.
All customer would need to appear in person in order to provide biometric data(fingerprints and digital photography), which is a simple and discreet procedure. This concerns the applications for the short-term Schengen visas (max. 90 days in 180 days).
For subsequent applications within 5 years the fingerprints can be copied from the previous application file in the VIS, eg. the fingerprints will be provided for the period of 5 years for any subsequent Schengen visa application (for example for the long-term multiple entry visa) .
Exemptions from the obligation of fingerprinting are provided for:
- children under the age of 12;
- persons for whom the collection of fingerprints is physically impossible;
- Heads of State and members of the national Governments (with their official delegations and spouses) if they travel for official purposes.
According to the decision taken by the Ministry of Foreign Affairs of the Czech Republic, effective since 18th of April 2011 third countries where the Czech Republic has a number of representative bodies, customer may apply for a visa only at the representative body within the consular district, in which they have been admitted for permanent or temporary residence.

- The visa fee is payable in Euros at the bank after documents submission. The bank charges the additional fee of UAH 45,00 for the bank services for each applicant.

- For employees - original certificate of employment, which contains the position, salary for the last 6 months, hire date and approved holiday period. - For entrepreneurs - license, registration certificate and latest tax declaration;
- For pensioners - pension certificate or document certifying the size of pension;
- For students and school children - confirmation from educational establishment with confirmation of leave of absence. Additionally, you may need to provide a student's record-book.
- For unemployed - a written signed explanation on the applicant's labor situation. Additionally you may provide a copy of work book with the previous official places of employment.

Application for a Schengen (short-term) visa can be filed within 3 months preceding the planned trip. Applications are in most cases reviewed within 7-10 calendar days. In individual cases, the review period can be extended up to 45 days and more at the discretion of the Consulate of the Czech Republic for any reason. It is recommended not to file a visa application later than 15 days before the trip, as it cannot be otherwise guaranteed it will be reviewed in time. The VAF has to be filled out in Ukrainian language with English transliteration or in English or Czech languages. The form needs to be signed personally by the applicant in the field 37 and on the last page. In case of the applications for multiple-entry visas there has to be a signature also under the declaration about the necessity to have travel health insurance not only for the first trip but for all the following trips.
For minors, the application must be signed by one of the parents or a legal guardian.

Journalist Visa

Overview
This visa category is eligible for customer who are qualified journalists and travel to the Czech Republic with the aim of journalism.
All customer would need to appear in person in order to provide biometric data(fingerprints and digital photography), which is a simple and discreet procedure. This concerns the applications for the short-term Schengen visas (max. 90 days in 180 days).
For subsequent applications within 5 years the fingerprints can be copied from the previous application file in the VIS, eg. the fingerprints will be provided for the period of 5 years for any subsequent Schengen visa application (for example for the long-term multiple entry visa) .
Exemptions from the obligation of fingerprinting are provided for:
- children under the age of 12;
- persons for whom the collection of fingerprints is physically impossible;
- Heads of State and members of the national Governments (with their official delegations and spouses) if they travel for official purposes.
It allows the citizens of Ukraine to travel to the Czech Republic and stay for a limited period of time of not more than 90 days in any 180 day period.
According to the decision taken by the Ministry of Foreign Affairs of the Czech Republic, effective since 18th of April 2011 third countries where the Czech Republic has a number of representative bodies, customer may apply for a visa only at the representative body within the consular district, in which they have been admitted for permanent or temporary residence.

- The visa fee is payable in Euros at the bank after documents submission. The bank charges the additional fee of UAH 45,00 for the bank services for each applicant.

Please note that as per the Embassy's requirement effective 1 April 2015 the Czech Republic Visa Application Centre will be not in a position to accept incomplete set of documents. Please prepare the full set of documents in advance as per the chosen visa category.

- Insurance should be issued by acbanked Insurance companies only, signed by an insurance agent and stamped.
Note: The insurance policy must be completed on the computer and then printed or filled on a typewriter. Insurance will not be accepted if it is filled out by hand, and if it contains at least one error in the passport data.

- A certificate or other document issued by a professional organization or the applicant's employer proving that the person concerned is a qualified journalist and stating that the purpose of the journey is to carry out journalistic work or proving that the person is a member of the technical crew accompanying the journalist in a professional capacity.

Proof of employment / activity and income.
- For employees - original certificate of employment, which contains the position, salary for the last 6 months, hire date and approved holiday period. The certificate shall indicate the name and position of the person who signed the certificate, registration and contact details of the company, also certificate should be certified by wet seal. Additionally, you may need to provide work book;

Proof of sufficient funds to cover expenses during the trip (only one proof is required).
- Bank account which confirms the adequacy of funds and transactions over the past 3 months. The bank statement must be submitted in the original and be stamped by the bank.
- Signed traveller's checks on applicant's name - the original and copy.
- Sponsorship - notary sponsorship letter, covering the entire period of the requested visa and bank transaction of a sponsor.
Attention: In case if the inviting person covers the expenses of the trip - a written request from the person or entity from the Czech Republic, verified by the police for foreigners.
Citizens of other countries are required to submit a registration or a residence permit on the territory of Ukraine. This document must be valid for at least 3 months after the expiry of the visa.
According to the Law of Ukraine "On Personal Data" agreement is signed personally by the applicant (for minors signature poses one of the parents or legal guardians). Please note that customer who apply through the representative must also prepare this document in advance.
In case of absence of this document the Visa Application Centre has the right to refuse to accept documents.

Employment Up To 90 Days

Overview
This visa category belongs to customer who plan to visit the Czech Republic for the purpose of employment for up to 90 days.
It allows the citizens of Ukraine to travel to the Czech Republic and stay for a limited period of time of not more than 90 days in any 180 day period.
All customer would need to appear in person in order to provide biometric data(fingerprints and digital photography), which is a simple and discreet procedure. This concerns the applications for the short-term Schengen visas (max. 90 days in 180 days).
For subsequent applications within 5 years the fingerprints can be copied from the previous application file in the VIS, eg. the fingerprints will be provided for the period of 5 years for any subsequent Schengen visa application (for example for the long-term multiple entry visa) .
Exemptions from the obligation of fingerprinting are provided for:
- children under the age of 12;
- persons for whom the collection of fingerprints is physically impossible;
- Heads of State and members of the national Governments (with their official delegations and spouses) if they travel for official purposes.
Citizens of Ukraine with Czech roots are able to apply at any Czech Republic Visa Application Centre without prior appointment. The applicant must provide proof of the given origin. Please inform the manager who will meet you at the Centre of your membership in this category.
According to the decision taken by the Ministry of Foreign Affairs of the Czech Republic, effective since 18th of April 2011 third countries where the Czech Republic has a number of representative bodies, customer may apply for a visa only at the representative body within the consular district, in which they have been admitted for permanent or temporary residence.

0 As per the Visa Facilitation Agreements between the EU and Ukraine the following citizens of Ukraine are exempted from the visa fees:
- close relatives (spouses, children (including adopted), parents (including custodians) of citizens of Ukraine legally residing in the territory of the Member States or citizens of the European Union residing in the territory of the Member State of which they are nationals;
- members of official delegations, who following an official invitation shall participate in meetings, consultations, negotiations, or exchange programs, as well as events held in the territory of one of the Member States by intergovernmental organizations;
- members on national and regional Governments and Parliaments, Constitutional Courts and Supreme Courts;
- pupils, students, postgraduate students and teachers accompanying them when they come to attend school, study or educational training;
- disabled persons and the person accompanying them, if necessary;
- persons travelling on humanitarian grounds (including urgent medical treatment) and the person accompanying, or to attend a funeral of a close relative, or to visit a seriously ill close relative;
- participants in scientific, cultural and artistic activities including university and other exchange programs;

- journalists and the technical crew accompanying them in a professional capacity;
- pensioners;
- drivers conducting international cargo and passenger transportation services to the territories of the Member States in vehicles registered in Ukraine;
- members of train, refrigerator and locomotive crews in international trains, travelling to the territories of the Member States;
- representatives of the religious communities;
- members of the professions participating in international exhibitions, conferences, symposia, seminars or other similar events held in the territory of the Member States;
- representatives of civil society organizations undertaking trips for the purposes of educational training, seminars, conferences, including in the framework of exchange programs;
- participants in official European Union cross-border cooperation programs, such as under the European Neighborhood and Partnership Instrument (ENPI);.
- participants in international sports events and persons accompanying them;
- children under the age of 18 and dependent children under the age of 21;
- participants aged 25 years or less in seminars, conferences, sports, cultural or educational events, organized by non-profit organizations.
Other nationalities:
- citizens of all other countries under 6 years of age.
- The visa fee is payable in Euros at the bank after documents submission. The bank charges the additional fee of UAH 45,00 for the bank services for each applicant.

Documents Required
Please note that as per the Embassy's requirement effective 1 April 2015 the Czech Republic Visa Application Centre will be not in a position to accept incomplete set of documents. Please prepare the full set of documents in advance as per the chosen visa category.
For Ukrainian citizens and other nationals legally residing in the territory of Ukraine, who are applying for the visa category "Czech compatriots" the list of required documents is as follows:

Visa application form.
- Completed and signed personally by the applicant (for minors the application has to be signed by one of the parents or a legal guardian).
- Filled in Latin letters.

Passport or other travel document.
- Passport must be presented in the original (including other valid passports). In case the international passport is not valid anymore, please provide the stamp in the internal passport or the confirmation that the international passport was cancelled.
- A copy of all pages with notes and previous Schengen visas.
- Not older than 10 years (extended passports are not accepted).
- Validity of passport should be at least 3 months after the visa expiration.
- Passport must have minimum two (2) blank pages.
- Original Internal passport and copy of all the pages with notes or original ID card and copy of both sides and original and copy of the Registration of Individual.

- Size 35 x 45 mm.
- White background.
- Not older than 6 months.

Original medical insurance policy.
- Coverage of 30,000 Euro.
- Franchise for medical expenses – 0 Euro.
- The grounds of action - all European Union countries.
- In case of applying for multiple-entry visa, the insurance policy must cover at least the first trip. In this case the visa application form should contain a third signature that certifies the necessity of having insurance policy for all future trips.
- In case of applying for a single entry visa, insurance should cover additional 15 days.
- Insurance should be issued by acbanked Insurance companies only, signed by an insurance agent and stamped.
Note: The insurance policy must be completed on the computer and then printed or filled on a typewriter. Insurance will not be accepted if it is filled out by hand, and if it contains at least one error in the passport data.
Attention: In case if the inviting person covers the expenses of the trip - a written request from the person or entity from the Czech Republic, verified by the police for foreigners.
The Consulate is entitled to require any other documents that confirm the data presented in the questionnaire, and to confirm the purpose of stay, financial and social status, health, etc. The Consulate also has the right to invite the applicant for an interview.
Please note that you may purchase medical insurance policy at the Visa Application Centre as well as order form filling by Visa Application Centre manager during submission process.
5. Confirmation of trip (original and copy).
- Сontract of employment or an agreement on work activity.
- Work permit issued by the Czech Labor Office in original only.
- Proof of accommodation
We would like to draw your attention that period of stay in documents that prove purpose of visit (contract of employment, work permit and proof of accommodation) should not exceed 90 days and should be the same in all documents. It is applicable for documents submission at visa application centres in Lviv, Uzhgorod and Ivano-Frankivsk.
Citizens of other countries are required to submit a registration or a residence permit on the territory of Ukraine. This document must be valid for at least 3 months after the expiry of the visa.
According to the Law of Ukraine "On Personal Data" agreement is signed personally by the customer (for minors signature poses one of the parents or legal guardians). Please note that customers who apply through the representative must also prepare this document in advance. In case of absence of this document the Visa Application Centre has the right to refuse to accept documents.
The Consulate is entitled to require any other documents that confirm the data presented in the questionnaire, and to confirm the purpose of stay, financial and social status, health, etc. The Consulate also has the right to invite the customer for an interview.

Photo Specifications
A Photograph submitted with the application for a visa should comply with the following specifications:
- Must be an up-to-date photograph (preferably maximum 6 months old)
- Must be a colour photograph printed on normal photographic paper
- Should be passport sized 3,5 x 4,5 cm
- Close up to your head and top of your shoulders so that your face takes up 70-80% of the photograph
- Taken against a light background
- Without sunglasses
- Without hat or other head covering unless you wear this for religious reasons
- Spectacles should not have toned or reflecting glasses
- The frame of the spectacles may not cover the eyes
- The mouth must be closed

Processing Time
Application for a Schengen (short-term) visa can be filed within 3 months preceding the planned trip. Applications are in most cases reviewed within 7-10 calendar days. In individual cases, the review period can be extended up to 45 days and more at the discretion of the Consulate of the Czech Republic for any reason. It is recommended not to file a visa application later than 15 days before the trip, as it cannot be otherwise guaranteed it will be reviewed in time. Please note that the processing time in General Consulate of Czech Republic in Lviv ( Visa Application Centre in Lviv, Uzhgorod and Ivano Frankivsk) will take 10 working days and not include 2 days for transportation the application from Visa Application Centre to General Consulate and 2 days for transportation the decision from the General Consulate to the Visa Application Centre.
Visa Application Centre has no right to influence anyhow the processing time of visa applications as well to accelerate the application.
Please note that you may be required to appear before a Visa Officer for a personal interview at the Consulate of the Czech Republic prior to a decision being taken on your application.
