Let us walk you through the process
Step 1: Determine the visa category for which you will be applying, depending on the purpose of your trip.
You can find visa categories as well as detailed information on required documents and requirements here.
Please remember that we are here to assist and help you through the entire visa application process but we are not permitted to advise or guide you on choosing a visa category or purpose of travel. Since our work is primarily administrative in nature, we have no say on whether you will be granted a visa and how long it will take to process, as this is entirely the prerogative of the Embassy/Consulate General of Republic of Poland.
Step 2: Complete you visa application form and affix your photograph. Visa application forms may be found on the page with information about the selected type of visa on the ‘Download Forms’ tab.
Visa Application submission to the Embassy/Consulate General of Poland is possible with a prior appointment at the Visa Centres in Minsk, Gomel, Grodno, Lida, Brest, Baranovichi or Pinsk or the Embassy/Consulate General of Poland. Submitting a visa application form in both institutions is equivalent.
Citizens of Belarus should apply for Schengen Visas and National Visas on a territorial basis in accordance to their respective address of registration:
At the Visa Centers in Minsk, Mogilev, Gomel - residents of Minsk region, Mogilev region, Gomel region and Vitebsk region.
At the Visa Centers in Brest, Baranovichi, Pinsk - residents of Brest region.
At the Visa Centers in Grodno and Lida - residents of Grodno region.
Holders of biometric passports should provide certificate confirmation of the place of residence registration.

Foreign citizens residing in the Republic of Belarus must apply for both a national and a Schengen visa to Poland on a territorial basis in accordance with their residence permits:
Minsk region, Mogilev region, Gomel region and Vitebsk region – at the Visa Centres in Minsk, Mogilev, Gomel and the Consular Department of the Embassy of Poland in Minsk.
Brest region – at the Visa Centres in Brest, Baranovichi, Pinsk and the Consulate General of Poland in Brest.
Grodno region - at the Visa Centres in Grodno, Lida and the Consulate General of Poland in Grodno.
Please note that foreign citizens residing in the territory of the Republic of Belarus must have a document confirming the address of registration (residence permit, certificate of family members, etc.).
Please note: in exceptional cases the consul has the right to request the provision of other documents as well or invite you for an interview at the Consulate.

Visa application submission is possible:
By the applicant himself / herself (for the adult applicants)
By parents / legal guardians for a minor child by presenting birth certificate. Visa Application Form for a child should be signed by both the parents. In case it is signed by one of the parents only, the other parent has to provide a written consent. In case there is only one legal guardian, same should be confirmed by sufficient documents.

For Visa Applying in Poland Visa Application Centers it is required to read, fill out and sign the Agreement for data protection. Please note that filling out of this form is mandatory for every customer.
Submit your application at the VFS Centre along with the applicable fees. The visa fee and service as well as additional services should be paid in Belarusian rubles by cash only at the Visa Application Centre.

For online tracking please provide unique reference number mentioned on the ICR issued by the officer of the Visa Centre during submission and your date of birth in dd/mm/yyyy format.

You can retrieve your passport at one of the visa application centres or you can have it delivered by courier. You may opt for Courier delivery service during application submission at the Visa Application Centre or after it if your passport is still being processed. Applicants can collect their passports from the visa application centres as follows:
15:00-17:00 (Monday-Friday, except Holidays)

Passport collection is possible:

By the applicant himself / herself (for the adult applicants) – please bring ICR issued while submission, passport copy and agreement

By family members or close relatives: please bring ICR issued while submission, passport copy and agreement
Marriage certificate or birth certificate and original of passport of the authorized person, written authorization consent. If personal details of such person had changed, respective documents should be provided.

By third parties: please bring ICR issued while submission, passport copy and agreement. It is also required to show authorization letter issued on the name of the authorized person stamped with a notary stamp and valid not more than 6 month from the date of issue. Additionally original of passport of an authorized person is required.

Representative of the company: please bring ICR issued while submission, passport copy and agreement. Additionally it is required to bring authorization letter from the company issued on the official blank of the organization including list of all the applicants for whom passport will be collected, authorization person position in the company. Authorized person should provide original of passport.
Applicants are requested to collect their passports from the Visa Application Centre within 30 days. If passports are not collected within this period, they will be handed over to the Embassy. Passports which are handed over to courier for delivery will be called off and returned back to the Embassy if delivery fails for more than 30 days. In both cases, applicants will have to collect their passport directly from the Embassy.

All services listed above are purely optional.
Important Information:
Using these services does not mean your visa application will be processed more quickly or that you will be issued a visa. Your visa application will be processed in the normal way by the Consular Department of Poland Embassy in Minsk/General Consulates of Poland in Grodno and Brest.
All additional services are paid only in the Visa Application Center in the national currency (BYN) in cash. All service charges are inclusive of 20% VAT.
