Documents Required
Documents required to apply for a South Africa visa

A passport or travel document valid for no less than 30 days after the expiry of your intended visit
Your passport must have at least one unused page for entry / departure endorsements
A completed Form BI-84 (application for a visa)
Payment of the prescribed fee
A yellow-fever vaccination certificate (if required)
Statement and/or documentation confirming the purpose and duration of your visit
Two color passport photographs
A return or onward ticket if you are travelling by air
If you have children (minors) travelling with you or joining in South Africa, you will need to provide the following additional documents
Proof of guardianship or custody or detail child’s birth certificate
Consent from the guardian in the case of an unaccompanied minor
Referral Letter from a Congolese doctor, specifying the nature of health problem and why the referral is being sought.
Appointment letter from a South African doctor with his or her full contact details in South Africa and the letter from the doctor should include the estimate of envisage cost of treatment and stay in South Africa.
Kindly note that patient going for more than 90 days must contact either at Embassy in Kinshasa or at Consulate General in Lubumbashi.
ID & national registration number of the Congolese resident in South Africa, if sponsoring the visiting patient.

Requirements for entering South Africa

Requirements for ordinary visa regarding the new regulations which came into effect on the 01st July 2005, by the immigration law 19/2004 from the Republic of South Africa

1. 2 recent passport photograph
2. New visa application form completed and signed by applicant (B1-84) in black ink
3. Payment of visa fee $45 (subject to alteration every financial year)
4. Copy of bio data page and original passport valid for 60 days and at least 2 free pages on passport
5. Copy of valid yellow vaccination card (make sure yellow fever vaccination card is valid for 10 years)
6. Declaration of the purpose of the visit (Medical/Business visits should have relevant accompanying letter)
7. Responsibility term/letter from the parents authorizing the minor of travels to South Africa and copy of the parents ID’s (Required even if the parent is traveling with minor)
8. Proof of funds, bank statements stamped by the bank and this statement has to show previous transactions and minimum of USD500 in the bank account or travellers cheques to the value of USD500 per person
9. Copy of traveling ticket, arrival and return with a reservation with the dates of departure and return, (in the reservation/booking it has to show the number of the ticket)or you can make deposit in the amount for the return ticket
10. deposit may be asked or requested/ repatriation fee
11. The signature on the form should be exactly the same as on the passport
12. In case of being a first time traveller to South Africa, you should present a hotel reservation via FAX (the reservation will not be accepted if done via internet), or incase of accommodation by invitation should be sent in English by the person inviting and signed by the same and be sent through with a copy of the ID as well as his legality in SA (Working visa, Student visa etc.)

In case of minors or dependents the person covering the expenses should include and attach in the documents of the minor or dependents a financial or responsibility term and the copy of the identification - ID.

The signature of the form on the minor should be done by any one of the parents.

All Above mentioned relevant documents need to be brought in originals for getting tallied at the centre with subsequent photocopies. Only Photocopies will be taken along with the application, giving originals back to the applicant.
